Abstract
The gold standard surgical option for radiculopathy due to cervical disc herniation is anterior cervical discectomy and fusion (ACDF), which has long been regarded as a suitable technique with a satisfactory fusion rate. However, significant perioperative and postoperative morbidity has also been reported. Therefore, anterior percutaneous endoscopic cervical discectomy (PECD) using a working channel endoscope has been developed to reduce open surgery complications and tissue trauma. This article aimed to demonstrate a cutting-edge technique of anterior PECD for soft cervical disc herniation. The surgical procedure consists of two parts: (1) a safe anterior percutaneous approach under fluoroscopic control and (2) selective endoscopic discectomy and decompression using specialized mechanical tools under an endoscopic view. The clinical outcomes are comparable to those of conventional open surgery, with the benefits of minimally invasive spinal surgery. With technical advancements in surgical instruments and optics, the anterior PECD technique will become more practical and safer in the near future.
